Estate Planning Questionnaire
Please complete the following questionnaire to the best of your abilities. Don't worry if you don't know certain answers or you have questions, the attorney's role is to review these issues with you and assist in making decisions.  If you have some of this information on an existing form, document, or balance sheet, you can attach that rather than repeating the information on this form. Ultimately, the more information we have, the more effectively we can advise you, and that's why this form is so important

Guardianship and Conservatorship Questionnaire
Guardianship: A guardian is someone appointed by the court to take legal responsibility and authority for the care of another individual who cannot protect themselves.
Conservatorship: A conservator is appointed by the court to take legal responsibility and authority for the assets of another individual who cannot protect themselves.

Probate Questionnaire
Probate: Also referred to as an Executor, a Personal Representative is someone, usually nominated under a Will, but always appointed by the court, to take care of the estate of a deceased individual as part of the probate proceeding. It is usually a short term responsibility that includes collecting the assets of the decedent, paying their debts, and then making distributions out to the beneficiaries.
